We sought out to develop a tool that takes the burden off of doctors and staff by streamline their practice. With features to improve patient intake, scheduling, encounter notes, and billing, ONLI Orthopedics is more than an EMR, it's a true practice management solution. Everything we built into ONLI Orthopedics has a purpose. No more unnecessary inputs that cause frustration and decreases efficiencies. With a built in AI and machine learning algorithm, the more you use ONLI Orthopedics the better it becomes at predicting diagnosis, billing and plans.
Our Story
Built out of frustration.
One high volume orthopedic surgeon, in one of the busiest practices in Atlanta, GA decided clinical documentation ends just before the last patient walks out the door. Not when documentation ends. Pop-ups, dropdowns, "next page." Something had to give. The necessity to create a better system started on pen and paper, imagining a better way to take a high volume clinic, integrate it into a workflow all Orthopedic surgeons could use and then deliver that product to you.

Started by two friends with one vision: build a practice management platform that truly respects and enhances the unique workflows of orthopedic specialists.
Timeline of Innovation
The Problem
Most days, Dr. Nathan Jové, an orthopedic surgeon in Atlanta, is running two races at once: seeing 80–100 patients a day and then struggling to document them.
EMR notes are a maze of clicks and templates, dictations come back 2–3 days later missing key details, and insurance denials can hinge on a single word—“cast applied” instead of “fiberglass cast.”
Realization: the problem wasn’t his pace or his standards; it was the system on his screen—never built for the real world of orthopedic speed or accuracy.
The Basement Beginning
It started in a basement, not a boardroom. Our co-founder, Nathan, partnered with Assid, an AI engineer and future CTO—on a radical idea for healthcare technology.
The conventional approach to Electronic Health Records (EHRs) is backward: develop a complex system, then force the clinical note to fit inside it. We flipped that model.
The idea was straightforward: start at the clinical note, then build everything else around it. Doctors already think in patterns—injury, exam, decision, plan—so the chart should follow that rhythm. By transforming structured data into intelligent, dynamic visit notes, they created charting flows that adapt instantly to the case, suggesting sensible next steps and eliminating repetitive, time-wasting input.
They initially set out to build just the clinical note writer. However, somewhere between whiteboard sketches and late-night tests, they saw the industry's flaw: other EHRs are so fragmented they make the workflow impossible and difficult for both providers and staff. They quickly realized that to build the note writer right, they had to build the entire Practice Management System (PMS).
No one was focused on making the whole system work together. That critical gap—the need for a unified, cohesive experience that connects the clinical note to the entire practice, from check-in to billing—is where ONLI was born.
COVID Shutdown → Full-Time Focus
The global pause triggered by the COVID-19 shutdown became ONLI's inflection point. With surgeries down, both founders go all-in on development.
This period was intensely productive and allowed them to build the core logic. They engineered a sophisticated diagnosis engine capable of logically tracking an injury through the entire arc of care, from initial plan to follow-up—culminating in the first version of their groundbreaking Notewriter.
This clinical foundation was quickly supplemented by core functionality, including a basic scheduler, seamless visit-to-visit continuity, and essential early billing components.
Before the product was even fully released, ONLI made its debut at AAOS, starting a five-year streak on the floor.
Live Beta & Core Validation
On November 17, 2021, the system entered its critical live beta phase. Dr. Jové used ONLI daily at full volume. This was the ultimate test of ONLI's speed, reliability, and real-world effectiveness.
Over the rigorous beta period, the system generated 2,483 patient notes, validating its stability under high demand. During this time, the team finalized the comprehensive billing and collections engine, implemented automated CPT–ICD matching, established robust links between surgery and clinic visits, and built dynamic operative reports that captured the full episode of care.
The result: ONLI consistently achieved orthopedic notes inunder 30 seconds. The company continued building momentum with sustained AAOS presence.
Official Launch
The official launch occurred on January 1, 2023, with the full practice onboarding of Atlanta Bone & Joint Specialists. This transition was a pivotal milestone, demonstrating ONLI's ability to seamlessly integrate advanced functionality.
This rollout required successful integration of multi-provider scheduling, imaging data, historical patient notes, and complex surgery data across the entire group. The biggest outcome: we proved ONLI could instantly scale from supporting a single surgeon to managing the comprehensive needs of an entire orthopedic practice.
ONLI continued to gather valuable feedback and spread our message at AAOS, marking our fourth consecutive appearance.
First External Practice
Onboarded of our first external practice: 3D Orthopaedics & Sports Medicine in Melbourne, FL.
This pivotal step proved that ONLI’s unified system was built for broader applicability, marking our first successful multi-state expansion and signaling the transition to genuine commercial growth.
This scalability was fueled by continuous refinement of our Practice Management System (PMS) functionality, including advanced features in patient intake, secure messaging, and complex referral management that further streamlined the entire clinical and administrative workflow.
By proving the platform's immediate portability and broader market appeal, we secured our footing as a proven solution. We capped this achievement by completing our fifth consecutive appearance at AAOS, engaging with the community and showcasing our latest advancements.
